Prior to your first Counseling and Care appointment, please download, print, and fill out both the Member Intake and Consent Form. Please bring them with you to your first meeting.
The cooperation waiver would need to be filled out once you have met with our Director of Congregational Counseling and Care and have been referred to Journey Counseling Ministries or another recommended counseling center. It outlines and seeks signature agreement of the collaborative and financial expectations in order to utilize the member assistance fund.
